I think this is the moment Haymitch knows she's in love with Peeta. It's why he tells the allies they have to keep Peeta alive in order to get Katniss as their Mockingjay. This pain she see's in Haymitch seems like it's over her choosing to die, that it's because he loves her and now he is going to let her die but that's because we didn't know he was lying yet. This pain is actually because he is about to lie to her, also because he realizes what's will become of her if something does happen to Peeta. He knows he can't guarantee Peeta's safety, that he is going to lie about who is he prioritizing.

"'It's not too late,' he says. Over his shoulder, I see my mother and Prim clutching each other in the doorway. We run. They die. And now I've got Peeta to protect. End of discussion."

Someday I'm really going to do a reread strictly to post every single Iconic Katniss moment with a masterpost numbering every single one. Katniss Everdeen may have moments of selfishness like every single normal person but she is not a selfish person. That is not an inherent quality of her character. And I will not shut up about this because I've had to listen to people tear her apart for years.
