We know baby Sam was a surprise but during any of the pregnancies, especially with What happened with Casey giving birth to Kayla, was there any thought of the antidote would cause any issues during the pregnancy?

Given that they tried unsuccessfully for ages to fall pregnant, did Kaycee or Ethan at any time think that their difficulties were due to the antidote or were they proactive in finding out before they tried if it would cause issues? And similarly to Tobias and Casey, were they concerned about any unknown effects during pregnancy?
Hey there! Thanks for the ask, Nonny! I just answered some of this here, but since your question is more detailed, I'll expand on it.
Tobias x Casey - I don't think it was a concern during the pregnancies. Casey was in good physical health at the time, and there were no signs that it would have caused issues. Even with her third pregnancy, the complications will stem from an Amniotic fluid embolism, which is unrelated to the attack.
However, when it comes to emotional/mental health, her PTSD will be impacted during her pregnancy as the normal fears mothers have will be heightened. Fortunately, she is very self-aware and will get the help she needs. But, the events following her pregnancy with Kayla will give me a chance to explore Tobias's PTSD that is triggered by nearly losing her again.
There will be long term physical implications for Casey... and it will be brutal. lol
Ethan x Kaycee - Since they struggled with infertility, but no cause could be found, they both thought it may have had some connection. They mentioned it to her treating physicians, but no link was ever detected.
While Kaycee and Ethan both struggled with anxiety related to PTSD after the chemical attack, it was actually more visible in Ethan during her pregnancy. In my HC, he returns to counseling to deal with his anxiety surrounding Kaycee's health and that of their unborn child.
