SRS Consult Date

Last week I submitted my patient intake forms to a surgery center. I was supposed to wait a week (it takes, max, that long to process a chart) before calling to make my consult appointment. Soooo... I called yesterday to do that. They hadn’t processed my my paperwork yet. Apparently I submitted the blank form to them. So I found the scan in my gmail and sent that to them. Incomplete... I printed the pages out double sided, but only scanned one side. They were super nice and gave me a tentative date contingent on sending in completed paperwork. I managed to get them a full copy of the forms last night. So...
