#course performance summary

I’m in psychology first year and i’m petitioning to have one of my grades changes to a W (Late withdrawl). It says I need a course performance summary, but then I read that I don’t need it if under circumstances such as being a first year and hardships that caused my grade. So will they understand and not ask for my course performance or deny my request because I don’t have one? (It’s summer how tf am I suppose to get one anyways)

No you HAVE to get a course performance summary completed for late withdrawal, incomplete petitions are rejected if you don’t have everything in order. You should re-submit your petition with the course performance summary attached. Hand in the course performance summary form to the course department, they can usually track down the prof and get it completed.

I understand for a late withdrawal you need a cps form signed by the professor. For a first year late withdrawal, do you not need the form? That's what I'm understanding from yorks website but I was hoping for clarification. Thank you.

A late withdrawal is a late withdrawal, there’s no first year late withdrawal or different kinds of late withdrawals.

To answer your question: any time you petition a course to be removed from your transcript you must always provide a CPS form.

Should i email the prof to book an appointment so they can fill it out for me? For some reason i feel really intimidated by this, should i explain that im booking this appointment for the reason of filling out the CPS? are profs usually okay with doing this

There’s no need for you to book an appointment or even meet the professor. Print the CPS form out, fill the portion that needs to be completed by you, and just drop it off at the course department (they have drop boxes for professors) and the department will get back to you when the prof has completed the form.

You need to get the CPS form to the prof in person because s/he needs to fill it out and sign it.

since it says "A petition must be submitted no later than three weeks following the development of the circumstances that haveprompted the petition." what circumstances would i petition under and would i need a course performance summary filled out? or can i do it without it?

I can’t tell you the circumstance because that’s what you personally went through when you were doing the course and should link to why you want to petition the course for a late withdrawal (that’s the regulation you’re petitioning under). You always need a Course Performance Summary when petitioning courses for a late withdrawal, if you don’t have it attached then the faculty will disregard your petition.

Hi i'm currently in 3rd year, I just petitioned to remove a humanities course that i failed in my first year. I need to get a cps form filled out but the problem is that the humanities proff is not teaching the course this year, and I cant seem to find her office location. Should i give my cps form to the humanities department? Would they give the form directly to the petitions committee or would i have to pick it up myself? Thank you.

Hi anon,

You need to go to the Humanities department and talk to them, they will be able to get in contact with the professor for the CPS form if you can’t. Once the form is filled out, the course department usually calls you to pick it up. No they won’t give it to the petition committee, it’s your responsibility to get the form.

I messed up so badly. I was doing a petition for two subjects and I couldn't figure out who to go to to get help with it. I waited to long and only now thought of asking here! I got a letter from my therapist but I don't know if I need to get a letter from the professors! it's been a month and I think its too late now :( isn't it?

You can petition for a late withdrawal to up to a year, but you also need to provide a reason as to why you waited past a month to submit the petition (I think LAPS is more strict about this).

If you’re petitioning two subjects then you need to get two CPS forms filled out from both professors. Having additional documentation is good, so submit your petition regardless just in case it gets accepted!

I'm taking an SU in class course and because of health reasons I haven't been able to attend weekly classes. The professor has still not posted midterm marks and hasn't answered my email. Is there another way to find out my mark to fill out CPS form

There’s no other way besides dropping off the CPS form at the course department, they will be able to get in contact with the professor if you can’t.
