Creates a `pronoun gender` column in a database

This doesn't leave room for neo-pronouns, obviously, but it's a step up from assuming you must have one of two genders, and also a step up from assuming that if you use that gender of pronoun you are that gender. I also want to ask for given name and full name instead of first and last name; that's how they're typically used anyway.
