If we presume that the pipis is some sort of living creature, and Tenna knows this and cares about it deeply-- perhaps in a sort of neglectful way, or perhaps the pipis just genuinely does not need that much care, I'm leaning towards the latter-- anyway, it would be devastating if it died in his absence.
It would be yet another thing that would make him feel like a failure, and a weight that would be particularly difficult to cope with. Out of all he has lost, I think the knowledge that a living thing he cared about died due to his absence would be the most difficult to cope with. He can adapt to and learn to live with his disabilities and PTSD, but a life cannot be brought back.
I'm a coward though so I think the pipis is probably still alive
