I think Mario did earn a medical degree, he seems like the kinda guy who has the work ethic to do that. If not a medical degree, at least a master's degree in chemistry.
Luigi is not a doctor... yet. I think he was inspired by his brother to pursue a career in the medical field too, but he didn’t start immediately. He could be in the middle of residency in Dr. Luigi (he’s an intern, likely for his brother considering their practices’ similarity.)
Wario is not a doctor either, he just wants to get his hands on the Megavitamins so he can sell them and earn a huge profit. Other than that I think he’d be a regular customer, considering the fact that he’s no stranger to illness. Which makes regular check-ups and treatment very awkward for both him and Dr. Mario.
All intelligent “generic enemies”/NPCs featured in Dr. Mario World have their own lives and likely did pursue a career in the medical field, some of them maybe even becoming a doctor before Dr. Mario. They’re also not the same “generic enemy”/NPC that’s playable/featured in other games. This includes:
Dr. Yoshi (debatable)
Dr. Toad
Dr. Toadette
Dr. Luma (debatable)
Dr. “Kamek” (likely just another Magikoopa and not the Kamek)
Dr. Lakitu
Dr. Goomba Tower (debatable)
Dr. Koopa Troopa
Dr. Boo (they could have been a doctor before they died, or they could have used their afterlife to become a doctor- see below.)
Since Dry Bowser and King Boo are deceased and literally have all the time in the world, they could go to medical school anytime they want and become a doctor as a means to do something with their afterlife.
(Good for Bowser and King Boo for not letting something like death stop them from pursuing their dreams.)
Everyone else featured in Dr. Mario World is not a doctor.
Peach, Daisy, and Rosalina are too busy with their lives to pursue a medical career. (Bowser also fits this category.)
(EDIT: Peach, Daisy, and Bowser also probably wouldn’t need to become a doctor. They’re monarchs; they could have doctors at their beck and call.)
There likely aren’t any medical schools on Kong Island, so Donkey Kong can’t pursue a medical career. (And I don’t think he wants to.)
(Yoshi could also fit this category, if the one featured in Dr. Mario World lives on the secluded Yoshi’s Island.)
(Same with the Luma, if they decided to remain in space instead of come down to Earth to pursue a medical career.)
Bowser, Waluigi, and Nabbit are villains and likely don’t want to pursue a career that involves helping people. That, and it’s too much work.
Alternatively, in Waluigi and Nabbit’s case, they could have wanted to pursue a medical career but didn’t have enough money to begin medical school.
Blooper, Dolphin, and Piranha Plant are wild animals. Wild animals don’t pursue medical careers, they’re concerned with surviving and reproducing.
(The Goombas mentioned above could arguably fit this category too, as they were described as wild animals in Super Mario Odyssey.)
Bowser Jr., the Koopalings, Diddy Kong, and all “Baby” characters are not doctors. I don’t think I have to explain why.
Unrelated but I think all “Baby” characters featured in the spinoffs (Dr. Mario World, Mario Kart, Mario Tennis, Mario Super Sluggers, etc.) aren’t actually the baby versions of the characters they’re based on. They’re just snot-nosed entitled brats pretending to be the real thing. And I hate them.
