i was wondering: do you/your followers have any resources for top surgery results on fat guys? i would say im somewhat fat, but even for me as someone whos a bit more """acceptable""" to skinny people i cant find any resources or results for tguys my size that arent "this is what botched top surgery looks like" ): i have my consult coming up this april after waiting about 20 months (!!!) and i have no idea what to tell my surgeon. i know a lot of top surgery methods dont turn out right on fatter guys and i had to settle with a general plastic surgeon cus the top surgery specialist i was originally going with retired like 4 months before my consult date (which, rude) so the more information the better (:
Hi anon! Firstly, congrats on the consult!! Thank you so much for this ask because it gives me an excuse to yap about this. I had my surgery with a general plastic surgeon so I can give you some info all about my own experience!
For fat transmascs, specifically, it can be really hard to find these sort of resources. Most of the information I got from r/topsurgery was stellar. Reddit isn’t letting me search literally anything right now for some reason, but there are plenty of people on there who got surgery while fat and have given their tips written out.
As for myself and some general tips…
What to think about/ask:
Depending on your chest size, the surgeon will usually suggest double incision or peri. Most fat people getting top surgery I’ve come across get double incision, but there is a chance that if they are small enough, you might be suggested peri/keyhole. (Yes, fat people can have small breasts.)
And with that, there’s a million things to consider.
How do you want your scars to look, if you do end up having them? Straight across, a little curved, slightly curved up at the ends?
Do you want flat, or non flat? Do you want a little bit of extra tissue left because you want your chest to look more like a chest typical of someone perisex expected to androgenize? (Many of them have non-flat chests, regardless of whether or not they developed breasts, and especially if they’re fat.)
Free the nipple? (i.e. Nipple grafts or nipple free?)
Are you prone to keloid scarring? Do you want to prevent them or talk to your surgeon about steroid injections treatments in case they appear?
For keyhole patients, do you want your nipples positioned in a specific way? Do you think double incision might be better for that, or do you prefer the possibility of no visible scarring?
All these questions might be helpful for determining what would feel best for you!
The dreaded “dog ears”
One thing that seems to be a big issue with fat top surgery-getters is dogears. (For DI, less so for keyhole.) So what the fuck are dogears?

For my screenreader users, dog ears are excess tissue on the sides of the breasts under the armpits that, for whatever reason, aren’t removed. This can occur when anyone is getting double incision top surgery (again, I’m unsure about keyhole), even if you aren’t fat.
Essentially, you’re lying down flat on the table the whole duration of surgery. Some tissue on the sides might be missed during removal, leading to anything from a small bump at the ends of the incisions to major amounts of excess tissue. Dog ears are a common fear for a lot of fat people getting top surgery, especially if their surgeon is inexperienced with fat bodies.
If you are worried about dog ears, PLEASE mention it at the consult with your surgeon!
My own experience!
I’ve already expressed some grievances with the general plastic surgeon I chose on this blog (Emily Ridgway, Bozeman, MT, USA). Well, “chose.” She seemed like my only real option, as top surgeons in Montana are incredibly limited. However, I’m still thankful I was lucky enough to get it at all. If you are fat, I absolutely wouldn’t recommend her, since the surgery center she operates out of has a BMI cap of 35.
Like the amateur graphic I created mentions, I have some very mild dogears. I’ve thought about a revision, and I might go through with that. But nowadays they seem so handsomely mine, y’know? And they’re a bit endearing. I might change my mind someday but right now they’re staying put.
On choosing a surgeon
Going into a consult and during research, it’s really helpful to find a surgeon you know has experience with fat bodies. And if you can’t find anything, that’s okay! Looking up the surgeon on r/topsurgery is a way to find a list of their results, not just the ones they curate. If you see almost all skinny bodies…use your judgment and maybe look elsewhere.
There are surgeons that do not have BMI requirements or need you to shrink yourself in order to get life saving medical care. If there are red flags or if they treat you poorly, you should reconsider. It can be especially hard to hear this if you’ve waited months to years for a consult, but it can save you a whole lot of pain in the long term.
