How I feel the creation of Otto Scaarbach went:
Artist: Here we go. He's all soft and round. Not a sharp edge to him. He has a soft friendly smile and little round glasses. He's all perfectly proportioned, too. His fingers actually look like they belong to him and none of his limbs are to long. To tie it all together, he has a German Accent. He is literally the least threatening thing ever made.
GDT (or whoever it would have been): Perfect!
Artist: If you don't mind me asking, what is his role in the show?
GDT: Well, he starts as Strickler’s lacky and then becomes a cult leader in season 2.

So I headcannon that Doc has a built-in German to English translator. So like he could be speaking in German and it gets translated to english, and everyone else speaks in English and it gets translated to German.
But what happens when it breaks?
Doc will not be able to communicate with people, because even if he can understand them, they can't understand him.
Example:
"Hey, Doc. What do you want for breakfast?"
"ein Kaffe und zier wurst, bitte."
