As the Destiel/supernatural community brainstorms these two, here is my own humble opinion:

Dean Winchester loves Castiel.

Dean would explore that side of their relationship if the world wasn't always ending and he wasn't a broken man.

Dean doesn't care about gay connotations.

He's got Chuck merging with his sister to the north, a nephilim nephew to the south, a deranged billie-grim to the west and his hunter friends disappearing to the east. He was a demon, his brother was a demon, one of his best friends was a vampire named Benny and he's allies with the queen of hell. Gay is just about the LEAST abnormal aspect of his life. Dean isn't repressed in his sexuality, neither is he unconfident.

Dean is repressed in his ability to LOVE

and be loved. Every one that he loves he categorizes as a "responsibility". Just rewatch season 1-15 as reference material. He wont allow himself to enjoy the fruits of loving and being loved in return. Because they could be ripped from him at any second, its better to not get attached. (His relationship with Jack for example.)

Castiel had his own fear that kept Destiel at bay.

If he said " I love you" full stop, SO HOMO, at any other point in their time together, it would prompt Dean to pull away from their unconditional devotion to each other. Because he knows Dean, watched him keep even his brother at arms length. So that small grace was what Castiel clung to and survived on. He only made that confession because, it would be the last time he saw him. He didn't HAVE to say it outloud, the emptiness doesn't have a fucking stethoscope to the wall, "did he say it yet?!" -No, Castiel knows it was now or never and Dean deserved to know that he was loved because Castiel CHOSE to love him, not because of obligation or blood. Castiel wants him because he is Dean muthafuckin Winchester. No one else will do for an angel of the lord.

They are in love.

And at this point, true loves kiss could very well be the answer to all their problems.
