Fellowships for Samira

does samira need a fellowship? No, and there’s probably some unpacking around why she wants to do one. That being said, I feel like there’s better options to the ones she’s considering.

EMS: The ED is the community’s safety net, and pre-hospital care is kinda like the starting touch point. In EMS, your goal isn’t to fix the problem, your goal is to stabilize the patient so you can package them and hand them off to the ED, who will have the actual resources for definitive care. There’s cut offs on what you can actually do pre-hospital, and very little tests to help you figure things out.

Speed is crucial in EMS, and in major traumas, you’re taught to follow the Platinum 10. Aka, you should be off-scene in 10 minutes or less. I think Samira could learn a lot here, really utilize her skillset in EM, and see patients in a way she hasn’t before. Really go out in the community and see people in their natural habitat and how those things affect them kinda deal, ya feel? Also, there’s usually a good amount of teaching involved and an opportunity to help improve pre-hospital care and the transition to definitive care. Also, Disaster Medicine is usually an area you can explore here.

Social Emergency Medicine and Population Health: This one lowkey highkey would be a GREAT fit for Samira. This program focuses around training physicians to address the social factors and inequities that contribute to disease and reduce disparities for vulnerable populations (yes I did rip a chunk of that off the Stanford website). But literally like… this is Samira just on the regular. It’s integrating social context, shaping policies for marginalized populations, and research on those things. It’s literally just the Samira Mohan fellowship guys.

Critical Care: I think it could be nice for her to have 1-2 years out of the ED and up in med-surg. You’re dealing with very vulnerable patients where this probably isn’t their first rodeo. It’s being on the other side of things, following up after handoff from the ED. Could be good for her even to return to the ED after, having more insight to the whole patient journey.

Global Emergency Medicine: Lots of focus around aiding developing healthcare around the world. EMS and trauma system development, rural health, education. Could also be a great chance for Samira to help marginalized communities, but unrestricted from just the US.
