You absolutely can banish a ghost by “accident”. Enough distress or discomfort, even without being properly channeled, can force a ghost out. But it also at least partially depends on the magical strength, and will, of the ghost. A ghost with a lot of animosity might linger regardless, while a ghost that meant no hard might be driven off.
It essentially is a contest of magical strength. Force of will, determination, staying power, however you wanna look at it, it all comes down to who has more power, and who's more willing to “give”.
When ghost monsters dust, it's absolutely possible they will come back as another ghost monster. It depends on how deep their identity as a ghost runs through their soul. That might seem a given, as they obviously held that form before, but if the soul loses enough fragments, and picks up enough others, it may very well become something else. Similarly a human ghost's remaining, linger fragments, could have latched onto the identity of a ghost strongly. So then when the ghost fully dissipates and the fragments scatter they could influence the other pieces they pick up enough that the human ghost reincarnates as a ghost monster. While human and monster souls are vastly different, the raw soul energy is neutral, and soul fragments tend to cling to the nearest, largest soul piece, regardless of classification.
(alright what is it with ghosts and bathroom doors? we had who we fondly called ‘Fred’ or ‘Fred the Perv’. the door latch worked fine. we had a hook style lock. we had door stops. but that bathroom door would slowly creep open no matter what. didn’t matter who was/wasn’t home or in the bathroom. the door would always find its way open. it was mostly just annoying so we ignored it but yeah.)
