Lazy Nezumi is a Windows tool that attaches to your art software and smoothes your mouse and pen tablet strokes in real time while you draw.
Okay, I finally tried it out now that I'm doing art on my desktop. It works great. Seriously. Install the trial and give it a go. It installs both as a stand-alone program for use with most (any?) programs and a Photoshop-native plugin. It also has several smoothing presets, from light line smoothing to full-on lazy mouse for sweeping curves. AND the developer just updated Lazy Nezumi on the first of this month, so it doesn't look like some kind of abandon-ware either!
