Yesterday I was on my 6th shift in a large ED and I had the fleeting thought, "Hm I haven't seen a gunshot wound yet."

The next trauma they announced? You guessed it. GSW.

[It was a level 2 trauma, a gentleman shot in the heel. No exit wound - sure enough, x-ray showed the bullet had lodged...in his calf.]

Do not fret - he was completely stable, very jovial and an overall pleasure to have in the ED. He'll be okay!

What’s the best way to ask my preceptor for a lob coat to keep my shit in (reference books, notes, phone, wallet, etc) without sounding like a cocky gunner asshole?

Hi, I'm a med student from abroad currently doing a pediatric surgery observership in U.K. I'm feeling a bit lost these days but I was wondering if you had any advice as to how to benefit the most from this experience? Like wether wards/clinic/A&E etc would be a more better learning opportunity?

Hello! That sounds exciting! The good news is that there is lots to see; so don’t worry about seeing it all. Just make the most of what you can.On the wards of most surgical specialities, ward rounds are fast. Time wasted on the wards is time that could be spent in theatre doing what surgeons do. So ward rounds are interesting, but they will be fast paced, and there won’t be much time for teaching or refleciton on surgical ward rounds. I do recommend seeing a few of these, because it can give you a good idea of all the sorts of things they treat, and you might be allowed to examine.

But if your tutor doesn’t mind, your time could be more excitingly spent elsewhere.

If you like surgery, it’s definiely good to get a little theatre time. Depending on the type of surgery, it might be challenging to get close to seeig things. The good thing about laparoscopic (keyhole) surgery is that it’s all put up there on a screen for you to enjoy! Paediatric surgery tends to take place on a very tiny scale, so without a screen it can be hard to observe operations because we don’t have viewing galleries like they do in the US. However, you might find a senior who will talk you through what is happening, which makes it much more exciting and educational.A&E is really useful because that’s when people come in acutely. You’ll learn a lot about how people present with various illnesses, how they are diagnosed, and you might get the chance to follow up that patient through their treatment pathway. Depending on your experience, you might shadow someone working there, or even clerk patients for them and then have them review and put a plan into action.Clinics are particularly useful for students because there’s a chance for you to sit in on consultations. You can get to examine patients, perhaps even clerk them before the consultant sees them, and then present to the consultant. You can ask lots of questions, because it’s one-on-one teaching. I think for your learning A+E and clinics are probably the most relevant place to be, but you can learn a lot from all of the above.Good luck and have fun!
