Edurant Offers Many Benefits Over Sustiva as First-Line HIV Treatment

Among those starting HIV treatment for the first time, Edurant (rilpivirine) is superior to Sustiva (efavirenz) on multiple measures, aidsmap reports.

Publishing their findings in HIV Medicine, researchers conducted a multicenter, prospective and observational study of 1,490 people with HIV who started their first antiretroviral (ARV) treatment with Edurant (786 people) or Sustiva (704 people) and were members of the Italian Cohort Native Antiretrovirals Foundation Study. All participants began the study with a viral load below 100,000.

Hello Dr Gallant. On a previous question you answered that treating H. pylori infection while on Odefsey is tricky. I asked about my HIV doctor and he said that the risk is theoretical since treatment is only for 10 days. On the leaflet of Odefsey states no co-administration with PPIs with no asterisks about short treatment period or taking the meds several hours apart. I want your opinion also since I am very reluctant to follow my doctor's view and start treatment for Hp while on Odefsey. Tx!

Your doctor is right that it’s theoretical in that it hasn’t been specifically reported, but it would still make me nervous. I would worry that going 10 consecutive days with subtherapeutic rilpivirine levels could be enough to result in viral rebound and resistance.

Hi Dr Joel, I am 39 Male living in UK. I have been on HIV medication Eviplera for last two year. My VL/CD4 are UD/670, no other health issue. I am tolerating well Eviplera and plsning to swith Odefsey as soon as available in UK. Understand from some your forum, complera has low barrier to resistance and resistance to complera will resistance entire NNRTI . Considering above point, a person in my situation should consider for change to new medicine like Genvoya, Triumeq or Trivcay?

If you don’t miss doses, don’t mind taking your meds with a meal, and have a consistently undetectable viral load, then you shouldn’t worry about the resistance barrier of either Eviplera (Complera in the U.S.) or Odefsey. The resistance barrier is more of a concern when we start people on treatment for the first time and we don’t know whether they’ll be good at taking medications.

I've been on Complera for several years with no complications. Do I ask to change to Odefsey because of the TDF vs TAF ? I am 70 years old .

I try to  avoid giving direct advice on this forum, but in this case it’s hard to avoid it. At the age of 70, you have impaired kidney function and decreased bone density based on your age. The tenofovir DF in Complera will only make that worse. Switching to Odefsey will make it better, so this is an easy decision.

Interesting the question related to counting milligrams. I was thinking about this as well. How toxic are drugs like Odefsey, Genvoya and Truvada. I take Genvoya, what can I do to minimize the toxin on my body, is there any specific food, exercise or something?

I’ve written frequently about the side effects and toxicities of drugs…very recently in the case of Odefsey and Genvoya. Scroll down.

Since Genvoya has no known drug-specific toxicities, there’s nothing special you can do other than to keep yourself generally healthy  and well hydrated and to make sure you’re getting your labs monitored regularly. Since it contains cobicistat, make sure you’re avoiding drugs that interact, including over-the-counter fluticasone (Flonase) nasal spray.

Continuing on the Odefsey vs Genvoya. I'm on Genvoya for 5 months and so far UD, not side effects. health. Thinking about the long run, should I stay with Genvoya or switch to Odefsey? Also what kind of medications I can't take because of Genvoya?

I think I answered the first part of your question already, comparing the relative merits of the two agents.  I can’t give you specific advice about what you should do.

As for medications to avoid with Genvoya, the list is essentially the same as the list for ritonavir (Norvir). The only over-the-counter medication to avoid is fluticasone (Flonase), a nasal steroid spray used to treat allergy symptoms. There is a long list of prescribed medications that interact. The drugs may not necessarily be contraindicated, but some drugs, such as statins and drugs for erectile dysfunction, should be used at low doses. The list is too long to go into here, but your doctors and pharmacists should always be aware that you’re taking either cobicistat (in Genvoya, Precobix, and Evotaz) or ritonavir (including Norvir and Kaletra) so they can take these interactions into consideration.
