The rate of people acquiring HIV that is resistant to antiretrovirals (ARVs) has risen in recent years, aidsmap reports. However, because for the most part, such strains of the virus have mutations that confer resistance to older HIV medications, their spread is not likely a significant impediment to finding an effective drug regimen.
Among those starting HIV treatment for the first time, Edurant (rilpivirine) is superior to Sustiva (efavirenz) on multiple measures, aidsmap reports.
Publishing their findings in HIV Medicine, researchers conducted a multicenter, prospective and observational study of 1,490 people with HIV who started their first antiretroviral (ARV) treatment with Edurant (786 people) or Sustiva (704 people) and were members of the Italian Cohort Native Antiretrovirals Foundation Study. All participants began the study with a viral load below 100,000.
The Food and Drug Administration (FDA) has approved Mylan’s Symfi (efavirenz 600 mg/lamivudine/tenofovir disoproxil fumarate), a single-tablet antiretroviral (ARV) regimen for the treatment of HIV among adults and children who weigh at least 40 kilograms (88 pounds).

Women in a global study began an HIV treatment regimen based on one of the two drugs after 20 weeks of gestation.
Viral suppression rates at delivery are higher among women who start antiretroviral (ARV) treatment for HIV after 20 weeks of gestation with a regimen based on Isentress (raltegravir) compared with those who take Sustiva (efavirenz)-based regimens, aidsmap reports.
Presenting their findings at the 2019 Conference on Retroviruses and Opportunistic Infections in Seattle, researchers conducted a study of 408 pregnant women in South America, Africa, Thailand and the United States between 2013 and 2018. Half the study population started ARV treatment between 20 and 28 weeks of gestation while the other half began treatment between 28 and 37 weeks of gestation. The median gestational age at the study’s baseline was 29.6 weeks.
I'm on Atripla & doc insist I take it in a.m. on empty stomach cause I eat dinner 1 hour before bed. I get Atripla buzz 2 hours after I take it and it impacts work. Can I take Atripla before bed? I've been on it for years with no other problems
Most people take Atripla before bedtime, but it doesn’t have to be taken on an empty stomach. It’s true that many people find they’re able to tolerate it better on an empty stomach, especially at first, but if it doesn’t bother you to take it an hour after dinner, you can do that. The patient is always the one who should decide how to take Atripla. It’s all about what works best for you, since there’s no wrong way to take it.
I'm about to start taking efavirenz. I'm a little scared because I've read it might cause gynecomastia and depression. Are those side effects rather common or are they rare occurrences? Tks, doc.
Nervous system side effects (vivid dreams, dizziness, concentration problems) are common early side effects of efavirenz. Depression and gynecomastia are much less common.
I've been doing well on nevirapine for almost 3 years. Now my doc wants me to switch to efavifenz. I asked her why and she said that with time nev. will be toxic to my liver and resistance is more likely to occur than with efav. Is she right?
No, not exactly. While it’s true that nevirapine has more liver toxicity than efavirenz, the toxicity occurs in the first months of therapy. If you’ve been on it for for almost 3 years, you’re out of danger.
As for resistance, it’s also true that failure of nevirapine can lead to broader cross-resistance within the NNRTI class than failure of efavirenz, but resistance only happens if you fail. You’ve been taking it for 3 years without failure, so resistance doesn’t sound like an issue.
I’m not telling you not to switch. Efavirenz is a better drug to start with than nevirapine, but you’re not starting therapy. You’ll probably also have to deal with some efavirenz side effects, at least in the beginning.
