Sarcophagus Slime
A mass of jellied amber ectoplasm wrapped around a charred, blackened skull, sarcophagus slimes are set as undead guardians in isolated and sealed locations. The earliest sarcophagus slimes may have been spontaneous or accidental side effects of mummification rituals, but once the cause and methods were figured out the creation of them became an intentional action.

Sarcophagus Slime Creature 5 Medium Ooze Undead Perception +10; motion sense (precise) 90 feet, no vision Languages Necril, plus the languages of its creator (can't speak any languages) Skills Athletics +13, Stealth +12 Str +2, Dex +1, Con +4, Int -4, Wis +1, Cha +1 Motion Sense The sarcophagus slime can sense nearby motion through vibration and air movement. AC 18; Fort +15, Ref +8, Will +12 HP 105 (void healing); Immunities bleed, critical hits, death effects, disease, paralyzed, poison, precision, unconscious, visual; Resistances acid 8 Frightful Presence (aura, emotion, fear, mental) 20 feet, DC 19 Speed 20 feet Melee pseudopod +11, Damage 2d6+4 bludgeoning, plus 2d6 acid Corrupting Gaze [1 action] (divine, visual); Frequency Once per round; Effect The sarcophagus slime focuses a pocked, melting skull at a creature within 30 feet. The target must immediately attempt a DC 21 Fortitude save. Critical Success The creature is unaffected. Success The creature takes 1d6 void damage. Failure The creature takes 2d6 void damage and becomes drained 1, or increases their drained value by 1, to a maximum of 4. Critical Failure The creature takes 4d6 void damage and becomes drained 2, or increases their drained value by 2. If this increases the target's drained value to 5 or more, the target dies and their corpse becomes a sarcophagus slime within 24 hours.

Sarcophagus Slime 3rd level spoiler [undead ooze] Initiative: +5 Necrotic Pseudopods +8 vs. AC – 8 acid or necro damage (sarcophagus slime chooses). Natural 14+: The sarcophagus slime can make a corrupting gaze attack as a free action. [Special Trigger] CQ: Corrupting Gaze +8 vs. PD (one nearby enemy) – 5 necro damage. Natural 14+: Unless the target succeeds on an immediate normal save (11+), they lose one recovery. Fear: While engaged with the sarcophagus slime, enemies with 15 hp or fewer are dazed and do not add the escalation die to their attacks. Flow Where it Likes: The sarcophagus slime is immune to opportunity attacks. Immune to Effects: The sarcophagus slime is immune to effects. When an attack applies a condition to the slime (dazed, hindered, weakened, ongoing damage, etc), that condition doesn’t affect it. Meet the Skull’s Gaze: When a nearby enemy attacks the sarcophagus slime and rolls a natural odd attack roll, the sarcophagus slime can make a corrupting gaze attack against that enemy as a free action. Resist Necro 18+: When an attack dealing necro damage targets the sarcophagus slime, the attacker must roll a natural 18+ or the attack only deals half damage.
