Nasopharyngeal carcinoma

NPC is a squamous cell carcinoma that is often asymptomatic until the tumor becomes so large it obstructs the eustachian tube (recurrent otitis media), causes recurrent epistaxis or nasal obstruction. It is strongly associated with diets high in smoked fish (high nitrosamine content in Mediterranean and Asian diets) as well as Epstein Barr Virus. It is often metastatic at the time of diagnosis and may even present clinically with only an enlarged cervical node.

Drug Metabolism Mnemonic

Phase 1:  cyp450 oxidation-reduction (+ hydrolysis); usually yields slightly polar water-soluble metabolites (often still active) geriatrics lose phase 1 first

Phase 2:  conjugation (acetylation, glucuronidation, sulfation, methylation) —> usually yields very polar renally excreted inactive metabolites. Geriatric patients have GAS (glucuroniation, acetylation, sulfation)
