 Aaa yeah! It’s honestly a silly little idea :’0 not really sure if it’ll make since haha, but i’ll do my best to explain it! Basically whenever Wilsons sanity starts to go down/ depletes his over all appearance changes, most noticeably is his hands and forearm! At his max sanity, 200%, nothing really changes because he’s doing fine and dandy but if it starts to go under 175% thats when it starts to happen, when his tips of his fingers turn black ( I always called it the “nightmare effect”, to myself that is. ) and it isn’t all too noticeable at first and he would really just put the though he had aside and worry about it later.

 - If his sanity dips BELOW 150%, things get really noticeable. His nails start growing and as they are doing so it wouldn’t be too pleasant so his hands would be in pain and really wouldn’t have the best time doing much of anything, cutting wood, picking up things, crafting things would be unpleasant. If his sanity stays at anywhere below 150% he would get used to it and find a way around the unpleasant feeling. Now if he’s fully insane, 0%, his hands and forearm at this point would fully black due to the “nightmare effect” and have fully clawed hands. He wouldn’t know what to do and his whole arm and hands would be in pain but more like a pins-and-needle type of effect all over for the x amount of time he’s insane.

 - Whenever he has his first encounter with this happening I would imagine he wouldn’t know what to do and would make desperate attempts to get rid of it, so a lot of flower picking and cooked green caps or coming close to chopping his whole hand or arm off. If there are other people around him, like Willow or WX, they wont be able to see unless they are insane or around the same sanity percent as Wilson, he would have to had to learn that the hard way. 

 But uh,, thats my little head-cannon in short. I bet many people have the same idea and sorry if its boring, generic or “cringy” :”0 just a silly little idea
