Do you consider someone with a third testicle to be intersex? A guy on a porn site I'm a moderator for (I check IDs and briefly interview people to make sure everyone uploading is of age) has three testicles, and one of the commenters said that's an intersex condition. Since I'm intersex, the guy in the videos turned to me to ask if that term is okay for him to use. He doesn't want to appropriate the language of a group he's not a part of. So I've been thinking about it, and a lot of his experiences - early puberty, being forced to go to the doctor and put on puberty blockers without being involved in the decision-making process, his parents discussing having surgery done on him to fix him that he only knew about because he eavesdropped on them, being mocked by peers for having differently proportioned genitalia - do map onto what intersex people I know have gone through. However, I'm unsure what others who are intersex think about this.
I would consider him at least intersex adjacent, in experiences. At the very least I would consider him a victim of the strict ideas society has about how 'normal' genitalia 'should' look. It caused a lot of self-esteem issues and insecurities about his body that he had to work through in therapy. While it did result in him using porn as a platform to show men that no, actually, women are not solely attracted to porn's narrow portrayals of 'attractive' and conventional genitalia and lots of people in his comments section have said that seeing him made them feel better about their own testicles being 'too big' or 'ugly' or 'abnormal'... like, that's him managing to spin straw into gold and turning an awful past of invalidation and bodyshaming into something positive. It's not as if that retroactively makes the years of being bullied in locker rooms for not looking 'normal' okay. He was still horribly bullied and talked over by his parents and not in control of his own body. Those are very intersex experiences.
(As a side note it's so depressing seeing perisex men say they think they're disgusting for not having 'normal' proportions. Sex Ed needs to be revamped to address these unrealistic ideas people have had beaten into them by society about their bodies.)
Polyorchidism is intersex, yes. It falls under the split/duplicated reproductive variations spectrum. We have it in our pinned intersex guide and we have it in our upcoming intersex visual guide (including the IGM segment, because polyorchidism often unfortunately faces IGM.)
(Full agreement with you about the sex ed thing. Sex ed needs to become inclusive of varsexuality and of not being "perfect.")

Josuke/Gappy Higashikata from JoJo’s Bizarre Adventure: JoJolion is an intersex and polyorchidism flag!
Jōsuke "Gappy" Higashikata from JoJo's Bizarre Adventure (Part 8) is (dubiously) canonically intersex, and he has polyorchidism! He's stated to have four testicles; however it should be noted within the source material the reasoning for this specific variation may or may not be considered intersex depending on your interpretation. And it's also spoilers, so be warned, I suppose.

#1308.2: “A collection of mysterious and unusual physical disorders:
Naevus pilosus—Enormous moles or birthmarks with great amounts of hair growing out of them. The "bearded woman" and "orang-utan man" seen at circuses and carnivals are usually victims of this disorder.
Plica polonica—The skin and nails turn spongy and black, the hair follicles exude a gluey liquid, and the hair itself becomes painful to the touch. Oddly enough, this disease is found almost exclusively among Polish people.
Polyorchidism—The presence of more than two testicles. This is the opposite of the condition known as anorchidism, in which a male is born with no testicles at all.
Saltatoric spasm—Spasmodic muscular contractions of the calves, hips, knees, and back that cause the sufferer to spring up or jump about uncontrollably every time he or she attempts to stand.
Xeroderma pigmentosun—Small yellow warts cover the entire body. The skin is wrinkled and abounds in strange white scales. This disease, quite rare, usually attacks children.”
True, but inaccurate. False. True. Terms have changed. False.
Some of the hairy women and men at circuses had hypertrichosis or wore fake fur suits. A rare few had nevus pilosus. Today, this disease is also known as congenital nevocytic nevus.
I’m skeptical about some of the symptoms presented for plica polonica, which is essentially a Polish plait, or a huge, scary dreadlock of hair. And by scary, I mean these kinds of dreadlocks will form on people who don’t wash or comb their hair. It can also form from a response to head lice. Maybe the symptoms described here also describe people with terrible hygiene? It’s not exclusive to Polish people either- I’ve seen an older Asian man with a very long Polish plait.
Polyorchidism is very rare, with only two hundred cases recorded. Most of the extra testicles recorded are functional. Anorchism is the lack of testicles, but it ranges from being born with female genitalia to being born with a penis and an empty sac.
Saltatoric spasms were a diagnosis doctors used in the late nineteenth century to apply to patients whose leg muscles jerked and jumped while they were standing. Today, the same patient would be diagnosed with myoclonus, meaning a disorder that involves involuntary muscular spasms.
Xeroderma pigmentosum isn’t like how it’s described here. Basically, children with this genetic disorder (not disease!) can’t be exposed to sunlight because they’re extremely sensitive to it.
And that’s it for the category Diseases!
