Writing from Carter's POV: okay, so he thinks of himself as Carter when he's at work or thinking about work. And when he's outside the hospital socializing with someone he works with he's still Carter in his head, but if he's pushing them away he's thinking of himself as John. Of course, Jing Mei has always called him John so she may or may not be an exception. At home or with family he's John, but he does like being Carter more.
