You can safely rename almost all Sims 3 content, .package files and .sim files, changing the file names to something else.  You have to leave the ".package" and ".sim" on the end the same though.

I think that you are talking about a sim that was packaged as a sims3pack?  I am not 100% sure how to rename them besides using a tool like Custard.  I had a sim I downloaded that was named in Russian and I tried to manually change but that didn't work for me but the sim loaded fine so I didn't give it much thought after that. 

If your sim is in sims3pack form and it won't install by you double clicking it, try just moving it into My Documents>Electronic Arts>The Sims 3>Downloads.  Then when your launcher opens you can just select it and install it.  Hope this helps.
