Bilateral Sagittal Split Osteotomy This young man had a protruding lower jaw (mandibular prognathism). He said that this was causing speech and eating difficulties. There was an overjet of 10 mm. It was decided to surgically set back his mandible. Bilateral bone cuts were made in the mandible. The nerve was mobilized with the distal segment thus allowing for free manipulation of the proximal segment. Extreme care was taken to preserve the inferior alveolar nerve throughout the surgery. A slice of bone 12 m in thickness was removed from the region of the bone cuts. His occlusion was checked and mandibular segments were fixed with plates and screws.
