Bilateral Sagittal Split Osteotomy This young man had a protruding lower jaw (mandibular prognathism). He said that this was causing speech and eating difficulties. There was an overjet of 10 mm. It was decided to surgically set back his mandible. Bilateral bone cuts were made in the mandible. The nerve was mobilized with the distal segment thus allowing for free manipulation of the proximal segment. Extreme care was taken to preserve the inferior alveolar nerve throughout the surgery. A slice of bone 12 m in thickness was removed from the region of the bone cuts. His occlusion was checked and mandibular segments were fixed with plates and screws. Bilateral Sagittal Split Osteotomy This teenager was born with a unilateral cleft lip and palate deformity and underwent cleft lip and palate repair as an infant. He had a normal maxilla in spite of his cleft deformity, but presented with an overjet of approximately 10 mm. He presented to our hospital for correction. Treatment planning was done and it was decided to surgically set back her mandible. He underwent presurgical orthodontics for decompensation of his arch. Bilateral bone cuts were made in the mandible. The nerve was mobilized with the distal segment thus allowing for free manipulation of the proximal segment. Extreme care was taken to preserve the inferior alveolar nerve throughout the surgery. A slice of bone 12 mm in thickness was removed from the region of bilateral bone cuts. His occlusion was checked and mandible was fixed with plates and screws. He expressed her happiness at being able to eat and speak normally following the surgery. It was decided to surgically set back her mandible. Presurgical orthodontics was done for arch decompensation. Bilateral bone cuts were made in the mandible. Extreme care was taken to preserve the inferior alveolar nerve throughout the surgery. The nerve was mobilized with the distal segment allowing for manipulation of the proximal segment. A slice of bone 12 mm in thickness was removed from the region of the bone cuts. Occlusion was checked and the mandible was fixed with plates and screws. Fine tuning of the occlusion would be performed using fixed orthodontics. Bilateral Sagittal Split Osteotomy This adolescent male has always had a very prominent mandible. Teachers at school had always complained that his speech was not clear. He had a skeletal class III malocclusion with an anterior crossbite of 9 mm. His parents brought him to our hospital for surgical correction of his prognathism. Bilateral bone cuts were made in the body of the mandible. The inferior alveolar nerve was mobilized with the proximal segment thus allowing for manipulation of the distal segment. Extreme care was taken to ensure that the nerve was not damaged during the entire surgery. A slice of bone 11 mm wide was removed from the region of the bone cuts and the distal segment was slid backwards and stabilized with plates and screws after occlusion had been checked. His facial appearance was completely transformed by the surgery. Bilateral Sagittal Split Osteotomy This young man has a very prominent mandible. It is extremely prognathic with an overbite of approximately 8 mm. He has had difficulty with eating and speech for many years now. Upon obtaining a consultation with a dentist in his hometown, he was referred to our hospital for correction. Bone cuts were made in the mandible and a segment of bone 8 mm in thickness was removed bilaterally. The inferior alveolar nerve was mobilized with the proximal segment allowing for adequate manipulation of the distal segment. Adequate precautions were taken throughout the surgery to ensure that there was no injury to the nerve. Occlusion was checked and the segments of the mandible were stabilized and fixed with plates and screws. The patient was very happy with the cosmetic and functional results of the surgery.
