And Dean thinks you can't love someone without hurting them. That he can't love someone new without hurting him family.

What if, the gasp at the end of the confession, when Dean realized that Cas loved him, was also Dean realizing what Cas had done in purgatory. (x)

After all, it is AFTER this gasp. After processing what just happened, that we see that Dean shattered.

Then again, just losing Cas is enough to do that to him. (x)
