I know this may be an useless headcanon but I think that Dr. Iplier have 4 wings. You know what? Because in the angel's mythology the healers angel's need more speed and be able to fly very high places and they need enough strength to for example lift a wall to save one of their buried friends, but that doesn't mean they are the most powerful, they always need someone to watch their back because once they are healing someone, they get very vulnerable, so yeah I dunno if is a good headcanon nheh.

@bing-iplier and I had a convo going about how Dr. Iplier’s wings are REAL fluffy white and a bit smaller than everyone else’s (so as not to get in the way of the patient) and they’re also real sensitive.
