In Super Mario 64, it is possible to cause Mario to die simply by positioning him between a crate and a slope. While most setups that achieve this (such as this one I posted previously) rely on him performing a Ground Pound to assume the correct position, the one shown in the footage can be activated even with a regular jump.

The underlying reason for Mario's death is that he is pushed out of bounds by the crate; this is also why he loses his hat. Whenever Mario dies from going out of bounds, a glitch occurs in his rendering routine whereby his hat, which runs a separate check if it should be rendered on every frame, skips the check and defaults to Mario's hatless state. The effect is purely visual; Mario keeps his hat after being ejected from the level.
