Harlequin probably sucks on fallopian tubes. He's just weird like that.

Adding onto this he probably plays around with lungs by blowing into the windpipe and watch it inflate like a balloon. Probably ties it off and jumps to see if the lungs pop or explode like overinflated basketballs. Doctor would observe for science and interest.
