Setting the tone: I am asking this as a curiosity, not out of malice. How did you come to accept/praise/defend "Narcissistic personality disorder", which I assume you meant as: excessive spikes of entitlement, arrogance, low empathy leading to low compassion in behavior.

-You can point and should point out if my assumptions/axioms, logical steps are incorrect from your perspective!

It seem to go against communism, where compassion is required for participating members to reach an ideal, utopian society.

This Hegel's dialectic, where 2 opposing ideas "thesis" are brought together to combine into a "synthesis", a complete better view of the facts.

I'm defending NPD as defined and constructed by medical literature, not any specific set of behaviours. I came to defend it because of people I know with NPD, and after learning about the many ways in which their existence is defacto criminalized. Even if NPD is all of those things and nothing else (something which is not true), it doesn't justify the extreme level of social isolation and medical or legal imprisonment.

Communism is not some utopian "let's all just work together" thing, it doesn't require a co-operative human nature to fucntion, it works on incentives. Workers seek to control the means of production because it benefits them, not out of an internal feeling of empathy.

Compassion has absolutely nothing to do with it, capitalism doesn't suck because capitalists are uncompassionate, it sucks because it creates incentives which work against the vast majority of the population. If you incentivize and facilitate "compassionate" behaviour, people will engage in that behaviour, regardless of their internal emotional state.

Start disappointing people and not backing out of it when they are upset, reject feeling ashamed of everything including of yourself, start saying No to things you do not want to do not just things you're scared of, do more of those things you're scared of but wish you could do, make your own plans and execute them, decide to do or not do something without basing it on who will Dislike it.

Free Will takes practice, and the chance of making someone somewhere Slightly or even Very Disappointed In You. But you're an adult and you can't be made to stand in a corner anymore.
