I keep messing up when I try to make things better. And it brings nothing but stress to the one whom I care the most for, the one I've been trying to patch things up since a while of little to no communication. I just get into this routine of comfort that when deviation occurs I think it's something I did and it's my fault so I enter this downward spiral. I feel them losing faith in me and I know it's me and not them.

It sounds to me like you have had a rocky past that has lead you down a path of Insecurity and Low Self-Esteem. You have found a way of coping that revolves around a set routine, that brings you peace and comfort when you continue to practice it.
Building a routine is a double edge sword. When you build a routine you help combat a lot of self-deprecating thoughts, however you get set in your routine and you lose sight of how to handle outward displacements in your routine.
The first thing you need to accept is that displacements are not always your fault. When you accept this, you begin to realize that the world isn’t turning because you say it so, and isn’t completely affected by how you live. Sometimes things just happen, life is unpredictable like that.
Secondly, work on your communication. Be open to the person you are trying to patch things up with, and let them know the full situation. Let them know you’re having issues right now, but that you are trying and you hope they will patient with you. This shows them proof that you care and that you’re trying to fix things.
Thirdly, and most importantly, work on your self-esteem. You won’t help anyone if you are constantly hurting yourself. Give yourself a chance and work on loving yourself again. Keep in mind that you are not an image in your mind, you create the image. Don’t set yourself unrealistic goals, start small and keep working until you change a little bit of yourself at a time. Change how you view yourself, change how you think about yourself, and pick yourself back up.
This isn’t easy, nor has it ever been easy, but when you start to believe in yourself others will begin to believe in you too.
