The top doodle shows Ichigo helping his little sister Yuzu, with her English literature homework. The bottom left is just a sad boy Ichigo and the bottom right is cryptid Ichigo.
Yuzu was one of the first people to notice something was really off about Ichigo, like the weird face mask he never took off, or the growing amount of bandages that were covering his body. The was also the fact that he kept eating alone in his room, and Yuzu had more than enough reasons to be very concerned about her brother.
So one day, while she's doing the laundry, she walks into his room only to see the skeletal teeth adorning the side of his cheek and splotchy white skin on his arm. Ichigo looks just as shocked as she does, clearly not expecting her at the moment, but he had been staying home so often lately this was honestly bound to happen.
Yuzu of course panics. She thinks her brother is dying, either that he's turning into some kind of skeleton. Thankfully Ichigo is able to help calm her down and convince her that no, he's not dying, and no he isn't becoming a skeleton, but he isn't quite sure what's happening either.
Trusting her brother with her life, Yuzu agrees not to tattle on Ichigo, and tries he best to help hide his worsening appearance. She's also secretly scared that if she does tell someone about Ichigo, they'll come take him away, and lock him up like some kind of monster.
((Yuzu helps braid Ichigo's hair when it starts to get really long. He wasn't sure about it at first, but after trying it out, he likes how it keeps his long hair under control and out of his face))
((The cryptid Ichigo is basally what some people manage to see in the middle of night around Karakura town. Despite how scary he looks, an 'Ichigo' sighting is actually regarded as a sign of good luck. The areas hes sighted at are often infested with the more creepy, calculating Hollows, who have been able to hide from the Shinigami for years. So when he kills them, all the negative and oppressive energy in the area is suddenly gone and replaced with a feeling of peace. Basically all the bad luck and sickness people had been suffering from in the area is now gone.))
((Ichigo's cryptid names are either Oni-Sama or Heiwa no akuma.))
((Ichigo's tail has a really bad habit of curling around the people he cares about or wants to protect. Ichigo tries to keep it under control but Yuzu don't mind, she actually finds it comforting.))
