He's self-aware of it too! It's part of why he's such a smug sly little bastard so much of the time, literally BDE confidence. His small body only exaggerates how big it looks, like it's disproportionate.
He's also very perceptive and has high self-control, so he's the sort of yandere that actually has a very good chance of winning someone over without ever having to make his obsessive tendencies known to you, and can refrain from any behaviors or words that would be off-putting or alarming. He's charming and charismatic and has an overall non-intimidating appearance and aura, not to mention has plenty of experience over the years in convincing people to perform all sorts of tasks for him, so it's very easy to get close to you and gradually manipulate and lure you into sex without having to try too hard.
And once he does, it's easy to convince you to keep coming back, considering how fully he fills you and how good that feels... and he's well aware of the power that fact holds as well, will openly tease you about it, makes mock-hurt comments about how you're just coming back for that and not because of him, aw... it's played off as a joke, but it actually does occur as a concern in the back of his mind, and so it's reassuring when you assure him that's not the case.
The only issue he will potentially run into is that he may accidentally mention or bring something up something he wasn't supposed to know... but he's so confident and skilled at lying he can outright gaslight you into believing that you told him that information.
