What would have happened if Elena had stayed pregnant?Would she have confessed to a hybrid? What would Klaus have done?
Or would she have tried to keep the pregnancy a secret and somehow give birth to the child on her own?

oh, I don't think there would have been any way to keep it a secret. she had hybrids visiting her often enough, who would be able to scent the pregnancy on her/hear the baby's heartbeat, and the first one to notice would have inevitably told Klaus. It's one thing to perform some simple kindnesses for their master's prisoner; another to hide something HUGE from him (when they are ALL sired to him, too). and we can assume they were under orders to report anything out of the ordinary to him, and this would be far out of the ordinary.
and this is if her blood donations weren't wack from the pregnancy, another way her secret could easily be revealed, as in swbs.
but this is a dark timeline. I think Klaus would have taken the child away from her at birth and she very well might never have seen the infant again. I think he would NEED to know who the child's father was-- the timing would be such that he would feel it to be a betrayal no matter what, and he would need to know just how deep the betrayal went. was she cheating on him with multiple men? The idea that it would be Tyler's would make this worse than if it were some human man's. now, the baby becomes not just a living, breathing symbol of her "adultery," but also, once Klaus finds out hybrids are fertile, a symbol of the future they might have had together if they had remained together long enough to fall pregnant together. Suddenly, Tyler Lockwood has something Klaus would have greatly prized. something he's stolen from him.
the baby would have been born less than a year into the exile. Klaus would still be reeling emotionally. even more so than when we saw him three years later.
I think he would show up personally for answers, get them, storm off again, and return after the child's birth. Of course, he would arrange for Elena's medical care during the pregnancy. It would be equal parts spite and greed that would motivate him to look after her in the coldest, most removed way possible by making sure her medical needs were met but not her emotional needs. Part of me thinks he would show up after the baby was born in the middle of the night and take the child while Elena slept, and she would wake up and the child would just be gone forever without even a personal explanation from him. He would send the child somewhere to be raised in safety so as to continue the Petrova line, but ensure that Elena would never get the happiness of raising her daughter. He would choose to rip her from her as an act of revenge for sure.
I'm not sure what would happen next. This is a very dark timeline.
I'm honestly not certain that Klaus would be able to forgive or move on from what happened in May 2011 (finding out about Tyler, daggergate, all of those revelations) if the child had been born and he was faced with the living embodiment of that betrayal. Part of me thinks the child would have cemented it permanently.
On the other hand, Elena would be VERY motivated to do whatever it took to get back in Klaus's good favor so as to get her daughter back. I'm not sure what her avenue to reach him would be, but given the smallest opportunity, I think she would manipulate and flatter and debase herself in ways FE prime Elena could never imagine, if only to get Klaus to care enough about her in order to reunite her with her daughter. I don't think she would ever stop trying. I don't think there would be room in Elena for romance with Rebekah, or for healing, or for anything other than getting back to the baby that was stolen from her-- even if it took years. (In a way, this is Elena, rehashing Katerina's sad fate.) Maybe Mikael would come for her, as in prime universe FE, and she would reunite with Klaus again in the same manner, but things would go differently between them. Maybe she would get her chance.
Things would spin out though. She would be even more distracted than she is now, and I'm just uneasy about Klaus in this scenario. I think it would expose a level of cruelty from him that Elena just hasn't seen, and probably will never see. Not to say she hasn't seen terrible violence from him-- just-- I think this would be the thing Elena really couldn't forgive him for, and this would also be the fuel to the fire she needs to manipulate him harder than she ever has before.
