There was a pizza place growing up that had this and Golden Tee. I remember it being quite the quarter muncher, but there was some weird dude who worked there that could get to the end screen.

Pretty wild for a game to bank on the tagline “you die in the game, you die for real.” But these were the edgier times in game advertising. Never played it myself, but the Game Informer article made it look pretty cool.
