I’ve noticed that a lot of people within this fetish (both feedees/feeders) seem to have a history of eating disorders or are currently struggling with eating disorders in addition to having this fetish.

I think throwing up as a reaction to shock or trauma is great. Characters are so overcome by emotion or pain that their body - not their mind - just reacts so violently because it’s too real.

Maybe the scope is still cold and the patient flinches a bit
The throbbing of a heartbeat offset by deep digestive gurgles and soft whines and little squelches
Or maybe there’s just disquieting, hollow silence, indicating a blockage or an infection
Doctors’ faces as they listen: nodding, concentrating, maybe concerned, maybe even rare, raw fear

The soft touch of rubber gloves, gently massaging lymph nodes, the abdomen, etc.
The sterile smell of a doctor's office and the crinkling of exam table paper.

Ooooh what are your favourite hunger scenarios? I'm a HUGE fan of a 'rags' character being forced to look on at lavish indulgence that they can't have- like looking through a bakery window or sitting in the cold rain with the scent of a nearby restaurant torturing them.

tbh it's so hard to choose a favorite!! there are so many (′▽`〃)
i am very fond of hunger scenarios involving multiple people, or one hungry person in the company of others who can hear and thus either tease/torment them about their stomach growls or offer them good eats/be concerned in response
oh, especially torture. for instance, a group of chaotic good/neutral types eating in front of someone kidnapped or left behind by the "bad guys" while interrogating them using their hunger against em. "you can eat when you tell us where your base is!" "i'll never break!" cue stomach growling
or less comical scenarios where the unfortunate character is literally being dangerously starved by a torturer, whether for information, mind control or just some sado-amusement. love it
also pretty fond of good old fashioned stubborn and proud characters denying their hunger (of course to be outed by their empty stomach) as others nag at them to eat. i write a lot of dystopian/apocalyptic stuff so usually it's a case of "i'll go hungry so my companion/group can eat more."
or much like your scenario, not being able to afford it! so they skip meals, subsist on next-to-nothing and deny being hungry or struggling financially until someone either intervenes or they pass out in public!

Listen here my bunch of bozos, let's take a moment to appreciate people with a small-ass capacitiy.
They're the kind of poor souls who will be stuffed at every meal, especially if they're used to getting themselves a normal-capacity-person portion.
Bringing them to a restaurant is guaranteed to result in them being overfull.
You may not be able to see the difference a meal makes on their frame, but a babe who gets stuffed when eating barely anything is so fucking adorable oh my god.
