The way that Miyuki's mother possibly thought he was a girl at first, hence why they have a feminine name with cutesy, softer Hiragana spelling as opposed to Kanji. And since they lived in Kowloon Walled City, I doubt the doctors there would be too knowledgeable about intersex conditions. Probably went unnoticed at birth. Just some speculation.
