Metaplasia. An important Topic in Pathology
Some Topics are Examiners Favourites. Recently much focus is on metaplastic changes occurring in Body.
Metaplasia is defined as a reversible change of one type of epithelial or mesenchymal adult cells to another type of adult epithelial or mesenchymal cell, usually in response to abnormal stimuli and reverts back to normal on removal of stimulus . It is an important Pathology Topic . Frequently questions are asked on Metsplasia . Squamous Metaplasia is a subtype of Metaplasia. A student should be aware of the Type of Places where this change can occur. Examples are :
§ In Bronchus in chronic smokers.
§ In gall bladder in chronic cholecystites with cholelithiasis
§ In prostate in chronic prostatitis and estrogen therapy
§ In Renal Pelvis and urinary bladder in chronic infection and stones
§ In uterine endocervix in prolapse of the uterus and in old age
§ In vitamin A deficiency

Herpes Zoster
A widely common viral disease .It is common in immunocompromised, Tranplant recipients and HIV Positive patients. It is an infectious disease, which affects the peripheral nervous system.
It is also known as shingles. Herpes zoster is an acute neuralgia confined to the distribution of a specific spinal nerve root or cranial nerve. It is associated with a characteristic dermatomal, painful and usually vesicular rash and occurs more often in the immunocompromised host.
The primary site of infection is Sensory ganglion, Dorsal root. The Trigeminal ganglion may be effected. It can effect any body part along dermatomes.
The feature is vesicles or Vesicular Rash .
Generally a single sensory ganglion is affected. Although the rash occurs most often in the distribution of the lower thoracic dermatomes, any spinal segment or trigeminal nerve division may be implicated.
The herpes zoster infections may involve the geniculate ganglion of VII CN. Herpes rash occurs on the eardrum and adjacent auditory canal.
It can become widespread in severe cases.
Acyclovir and related Drugs are useful. The Complication of Post herpetic Neuralgia is a serious issue as it can be intensly Painful and responds to Phenytion , Carbamezapine like Drugs .
