this is before her mom asked Amane to call the doctor. she already has the phone out and the number 119 (Japanese emergency number, like 911) punched in. she was already planning to call for help

Amane knows exactly how brutal the punishments will be for this. she knows this is extremely forbidden and she will be tortured for it, likely by the very mother she is trying to save. and she still was about to call. she was willing to go through drowning, electrocution, beating and more - physical abuse so brutal it could have very well killed her for her first, likely "lesser offense" - for even the chance to save her mother

it's only when her mother breaks the doctrine first (and reveals herself to be a hypocrite) that Amane attacks her. because that's what she was taught to do. that calling for medical help (or asking someone else to) is a crime worthy of the death sentence
