And now, without further ado... THE ANSWERS. I'll be explaining this in a way that makes sense for those who are reading fandom-blind.
Paparazzi helps celebrity flee his security detail. - HAPPENED
The least voted option! Yeah, I mostly put that in for those voting fandom blind. For the few who may not know, after Jack Atlas (the former Duel Champion and King) loses to Yusei in the Fortune Cup and breaks his arm, all sorts of reporters go fleeing. One of them, Carly, who works in a tabloid but wants to be an actual serious reporter, realizes there's some kind of conspiracy/mystery going on based on footage and breaks into Jack's hospital room. One duel against a possessed cop later, and she's helping him hide out while his former personal assistant and said cop (now non-possessed) are looking for him. The whole thing is actually hilarious, especially the roller coaster scene.
Cop gets bullied by black woman into a redemption arc. - HAPPENED
This screenshot speaks for itself. She basically forces him to do chores for the orphanage, which is in a nasty area to put it mildly, in order to teach him some humility considering he was power-tripping HARD in previous episodes (hell, he was power-tripping in Season 0, Martha finished what the Pharaoh started). It works.
Someone in the reblogs commened she's more like a Latina, and I can't argue... (the anime never clarifies her ethnicity). So my bad for that.
Grandpa races motorcycle on foot. - HAPPENED
I didn't find an actual screenshot but I found a shitpost and it more or less covers it.
This is Jose, also known as Jakob (can't remember which version calls him what) and he's an android. Still not sure why he chose to RUN the first couple hundred meters before riding his D-Wheel, intimidation factor? Anyway, that was a meme for a while.

Trick question, in was on-screen only in the original! The dub cut that moment! You probably shouldn't boast about how you conducted experiments on a teenage boy that resulted to his death while his older sister literally has an evil lizard god that uses human sacrifices to power itself on her side...
Man gets bitch-slapped by a ghost while flying. - HAPPENED
This has to be a dub vs sub thing... This HAS to be a dub vs sub thing... because otherwise how could you all possibly forget...
About the FUDOU BITCHSLAP
This happens during Yusei's duel against Z-ONE, which by the way happens on the air, courtesy of Aporia giving Yusei's motorcycle wings so it could FLY. Yeah, he rides a winged motorcycle over the city. About halfway through the duel Z-ONE dumps his backstory onto Yusei and Yusei freaks the fuck out, so the ghost of dear Professor Fudou here uses his last chance to talk with his son ever to literally slap some sense into him.
I can't believe this got so many votes. I was expecting more votes on this than average since the season never got dubbed, but OVER 20%???
Sore loser mugs "Grim Reaper" to duel the protagonist. - HAPPENED
Yusei's first Fortune Cup duel, in Season 1. One of the other participants was a mysterious man named Shinra, who wore a dark face-concealing cloak and had a moniker of being the Reaper for being that intimidating I guess. The sore loser is the former champion before Jack Atlas, who is still salty about losing his title to him. So he sneaks into the tournament, and overhears one of the organizers telling this "Grim Reaper" about how Yusei defeated Jack. Sore loser decides that he HAS to duel Yusei, so he mugs the "Grim Reaper" off-screen, steals all his clothes except for his underwear, and takes his place in the tournament disguised under the cloak only to throw it off for a dramatic reveal. Apparently Yusei finds it funny.
Criminal gets hired by a school to discipline a teacher. - HAPPENED
One of the filler episodes in Season 2, with the criminal in question being Yusei considering the sheer number of cyber crimes he commits (as well as the numerous other charges on his sheet as well as serving jail time). Yusei works as a repairman during this season, so Duel Academia (at least the local department) hires him to "fix" something. He assumes it's a machine and shows up with his toolbox. What actually happened is that one of the teachers was power-tripping hard by calling the cards the students had weak and worthless, and since Yusei's deck is full of such cards, Aki (a friend of Yusei's who attends the school) suggested to the principal to have Yusei teach him a lesson. Principal accepts.
A butler is erased from existence for vigilantism. - DID NOT HAPPEN
The correct option, and the one who gathered the most votes! Congrats to the (almost) 30% of you who chose right!
I have to explain my "wrong answer" policy on this: use elements that already exist in canon and fudge some things to make it look convincing. There really IS a vigilante butler in 5D's, he works for Sherry and helps her do shit like break into the police HQ as part of an investigation on who killed her parents. But he does not get erased from existence. The time-travelling villains remove his participation from the World Cup tournament to take his and Sherry's place, the butler is perfectly fine otherwise.
Protagonist gets delivered a card by teleporting to a meteor. - HAPPENED
I have to admit I often get confused between comet, meteor and asteroid but I'm pretty sure I got it right. Anyway, this is how Yusei got Shooting Star Dragon! Bruno (in his Dark Glass disguise) tries to teach Yusei the Clear Mind technique, which enables Yusei to teleport with his D-Wheel. He never actually uses that to its full potential, which is disappointing, but oh well. The point is, once Yusei achieves this, a giant stone tablet suddenly falls from space, and lands outside the city, and at that moment Yusei teleports to the location that stone tablet landed. The tablet transforms into a card, and it becomes one of Yusei's most-used cards from that point on. Accel Synchro, baby!
Clown gets hounded across the city over instant ramen. - HAPPENED
Technically, he was hounded for stealing a flash drive with a super important program that could not be recreated. But Jack Atlas is less concerned about the program and more concerned that the guy ate one of his precious instant ramen packs, so while the others worry about the program Jack vows vengeance over the ramen. Just... watch the episode, it's ridiculous.
Man goes terminator at the sight of French woman. - HAPPENED
That thing I mentioned in the butler entry about breaking into the police HQ? Well, Bruno finds Sherry while she's about to access the police mainframe to analyse a Plot Sensitive Object, and the result is a fist fight. She manages to talk him out of it.
Viking god gives man radar vision. - HAPPENED
World Grand Prix arc! Team Ragnarok is a group blessed by Norse gods, and each of them has a glowing rune in one eye. In a flashback, one of them uses that eye to check for rocks and other potential obstacles under the water when steering a speed boat so they don't sink. We don't find out if all three of them can do this.
