Local Hospital System Doesn't Know About The Existence of Non-Ashkenazi Jews

This is a clinical genetics intake form, not a general ethnicity survey. I'd bet it's at an OBGYN office.

The form isn't doing an ethnic survey or trying to get to know the patient, it's trying to figure out if the patient needs Ashkenazi-specific genetic carrier screening - it isn't forgetting or invalidating anybody.

When my first child was conceived, being ashki meant that genetic screening was in order.

Ashkenazi Jews carry elevated carrier rates for a specific cluster of recessive disorders (including Tay-Sachs, Gaucher, Canavan, and familial dysautonomia) at frequencies high enough that ACOG and ACMG have issued screening recommendations specifically for this population.

The Ashkenazi founder-effect disorders are largely specific to that population's genetic bottleneck.

Depending on the speciality or condition being screened for, Sephardi or Mizrahi Jewish heritage might be appropriate as separate categories having distinct clinical relevance.

It's not an elegantly designed form, but it also is not a reason to take offense.
