Fun fact: It is so rare to see groups of rural physicians, particularly of the full-spectrum family physician subspecies, that there is no official name for a group of them - we’re too busy covering the clinic, the hospital, L&D, the ER, and the nursing home to congregate in groups of more than two, and even then, it happens just long enough to deal with a life-threatening emergency
The only possible exception occurs every 2-3 years, where we all meet for five minutes so everyone can try to avoid being made Chief of Staff and the successful docs immediately flee the meeting, leaving the new Chief to the cruel fate of having to do extra administrative work

This number called me asking how I felt about vaccination for respiratory illnesses and said they were from the cdc, but my phone initially silenced the call as junk. Yeah I’m standing in an icu I’m the doctor obviously I fucking love vaccinations for everything because they decrease the number of dying people I have to try and keep from dying. Fucking obviously.
I had to hang up early because one of my seven flu admissions was trying to die.
Why is the CDC doing this? Why is it screened as junk? Why are they calling me, of all fucking people?
Extubated a woman just to have her immediately a fib rvr to 190s and flash and require immediate re intubation.

There are so many cases I could talk about - great learning cases - but it’s been nonstop and I’ve been so swamped, so here’s some highlights:

Caudate mass with neuromuscular weakness and renal failure unknown cause and it improved without steroids
Wounds wounds wounds; the exposed skull conundrum (and several cases of what I now call “flap hat”)
Post extubation stridor; ut oh spaghettio where the fuck are the racemic nebs
I’m applying to work with another ID dept of another big academic hospital and I told them I’m quantiferon positive and they…. Asked for a third repeat quantiferon from this year?
I’ve been positive for years guys why? This is a lifelong positive, not a fluke. It’s because … South Africa. I worked in TB hospitals. This is real. My immune system has known mycobacteria. Stop making me repeat this expensive annoying blood test that will never change result. You’re the ID dept, you know this. Just accept my normal chest x ray and move on.
