At a coffeeshop
Two hours ago I was in a trauma room doing compressions on a toddler. The mother’s screams mixing with the sounds nursesdoctorstechs calling out, the beeping monitors telling us what we already know. Now I’m sipping a raspberry vanilla latte out of a mug with red birds riding snowflakes listening to some 80s music. The baby inside of me is kicking. My hands still smell like gloves and hand sanitizer.
This job is completely surreal.
It’s hard to see what we see, and then go back to ‘normal’ life, surrounded by people who will hopefully never have to witness what we’ve seen, much less play an active part. Sometimes the ordinary is so jarring and suffocating that you almost can’t stand it, at other times it’s like a warm blanket, insulating you from the hurt. I hope you can take time to process, and to make peace with what happened. And to rejoin the outside world.

Same. While everyone is at home sleeping at 0300, I am desperately fighting to keep my patient alive as she decompesates faster than we can intervene. Her daughter is crying in the waiting room, terrified to see her mom with all the tubes. We are placing an emergent vascath. Her lactate is 20. We are losing...
My face when my patient's lactate jumps from 10 to 20 in three hours.

Anesthesia broke my patient
We got my lung transplant extubated and the acute pain service comes by to place an epidural. I'm holding my patient on the side of the bed, she's been extubated all of 5 minutes.
Epidural placement goes fine, they give the test dose and BAM! MAP of 29.
It took three pressors and multiple boluses of epi and phenylephrine to stabilize her.
So anesthesia broke her, but at least they helped me fix her too.

When you give a bolus of propofol/dilaudid/fentanyl and they tank their pressure.
Except you can’t bolus Propofol 🤗 specifically because it causes severe hypotension enough to stop pulses
Pretty sure that we 10000% push propofol during procedures such as intubation all the time.
Also we do bolus it in the ICU, mostly when people are going a little crazy pants and grabbing for the tube, central line etc.
Saw an ED nurse push propofol for an ankle reduction…. and can’t it be pushed for conscious sedation? We don’t do it on my floor

Yup, we do it all the time too. Can't have my ECMO patient sitting up and decannulating themselves, now can I?

Took my first balloon pump today, emergently placed at the beside. The patient and I both survived, even thought my patient was cranky as hell when he came around.
But cranky ain’t dead, so that’s a plus.
