if anyone feels bad that their fic might contain medical inaccuracies, be comforted to know that whatever creative liberties you take with your medical scenes can never be worse than the one I just read. where one character tries to stop the bleeding on their friend's head wound by putting a tourniquet around their neck

Weapons. Trained, tested, forged in steel and fire. Failure is an inevitability that ends in death. Pain should not be felt--it should be recognized, familiar, and inconsequential
Martyrs. In the form of servants and princes, of leaders and underdogs. If blood is necessary, the martyr will lift their hands and offer it all
Shields. Like tempering a sword, but only to bear and not to lash out. Wounds are medals--not symbols of pride, but symbols of worth. A pretty shield is useless; scars mean a job well done
Experiments. Raised on the cold comfort of a lab table. Restraints are only necessary when they're not in their right mind. Is it honorable, to be twisted beyond recognition? Or is it just a necessary evil?
Monsters. Cruelty, caution, and regarding one as a creature beyond reasonable thought is tempering in its own right. But if you keep a leash at the right length, perhaps the massecre won't reach you. One can hope.
Idols. Pretty face, pretty name, pretty hands around their shoulders and throat. There to seduce, manipulate, force any feeling to come to the surface and twist it to their favor. Any genuinity stays locked behind the guilded cage that surrounds their pretty little heart
Trophies. Status and wealth and the traditions that keep someone at their heels, on their knees, to display and serve and decorate one's ballroom.
Sacrifices. Drenched in honorable clothes, prepared and adored and cleansed. The gift of hope at the cost of one's life. Is it taken with no fight? How can you escape the ropes you were born in?

Thoughts on the "oh no we have to fight while in formal wear" trope? (I just finished listening to the gala episode in RWD S2)
Almost impossible to get it wrong. You can learn a lot about a character by how they deal with that situation, and since it's an unusual scenario for them right from the jump, it's a characterization goldmine.
A character who values their formalwear because they never get a chance to dress up this nice getting absolutely ENRAGED when the scuffle leaves it damaged
A character who values their formalwear because they ALWAYS dress nice but this is their chance to dress REALLY nice going hulk mode at the slightest nick or misplaced cufflink on their best duds
The character who's been annoyed at having to dress up the whole night immediately taking the opportunity to split every seam/rip off the sleeves/ditch the high heels
A character so precise and careful in their fighting style that they come out the other side just as flawless as they went in
The character who heard "no weapons on the dance floor" and was like "cool, only what I can hide in this outfit"
One of the theoretically absent characters actually snuck in as a waiter or in a masquerade mask and pops up at the eleventh hour to save the day
Someone's intricate hairstyle coming loose mid-fight, but like hollywood-loose where instead of being an awkward mess of bobby pins and half-braids it just goes full flowing and free

presyncope sensations
got a character you wanna make pass out (or very narrowly avoid it)? good! here's some thoughts:
head: light-headedness, dizziness, etc are all what come to mind, but there can be an overwhelming sensation of pressure. it's like a soft blanket used for violence, to smother their brain into submission.
eyes: never forget the classic fade to black or tunnel vision, but make sure to remember seeing stars! tiny bright floaters before and after passing out, and even if they don't experience a complete syncope. sometimes it can feel like their eyes are being squeezed in a vice grip, and they may end up hunched over, hands pressed to their eyes in a desperate attempt to relieve the pain as they focus more on that than the fact that passing out is imminent.
mouth: maybe it's dry, but the sensations of numbness and even tingling can dance across the tongue, lips, and teeth. maybe lead has replaced all of their muscles in their tongue, and speaking feels like a struggle as the mouth and brain shout across a phone line that's quickly losing reception.
ears: when the world gets put on a muffled semi-mute, that can be distracting. odd distortions happen, with sounds making it across the barrier but not quite lighting up the parts of the brain that would acknowledge the importance of what's being heard. yeah, it can be like being underwater, if underwater also came and went and let sounds cross at random.
chest: no matter how much air is frantically gulped down, it's never going to be enough. there's a hunger for oxygen, a hunger that simply isn't sated no matter how they try. it burns, it aches, it begs to be acknowledged and taken care of but nothing seems to actually fix it.
abdomen: imagine the feeling of a roller-coaster drop, but it's just the internal organs taking the plunge. it's an overwhelming rush of sensation that really shouldn't be there, and it can get overwhelming.
limbs: the overwhelming burn of a muscle that's at the end of its rope pops up before sensation is lost. going limp and numb would be a blessing compared to the shriek of the tissues insisting that something is very wrong. it seems to be burning the bridges that tell limbs what to do; the response time is incredibly slow. limbs can also give out before the brain does, leaving somebody on the floor before they even fully lose consciousness.

common literary & character tropes
Beware the Quiet Ones: When the character who is hardly ever upset about something, suddenly raises their voice, the world turns upside down and seems to come to an end. There is an unleashed raging or cold speech of epic proportions that not even the most demented character in the story would want to sit through. This rage is almost always expressed verbally, though violence can also be included. Another version could be when the heroes' team is in low spirits, and The Quiet One, fed up with all the sulking, throws the table (or something else) to the side and gives a Rousing Speech to their comrades.
Elective Mute: It turns out that a character assumed to be unable to talk actually can speak, they just choose to be silent most of the time.
Emotionless Girl: An enigmatic female character who appears to be entirely emotionless. Whether she actually is emotionless depends on the story and often on her level of characterization.
Heroic Mime: A hero who never speaks.
Silent Scapegoat: Somebody who willingly takes the blame for everyone else's wrongdoings.
Suddenly Speaking: A character who was initially silent eventually reveals that they can speak after all.
The Quiet One: A character who does speak, but not as much as the other characters.
The Stoic: Quiet demeanor tends towards the brusque or outright rudeness, though there are a few polite Stoics. Some stoics may try to give the impression of a lot going on inside, cultivate an air of mystery and confuse other characters with cryptic one-liners. The Stoic sometimes displays emotion when under extreme stress or in other highly emotional situations, but their usual repertoire consists of mild boredom, detached interest, Dull Surprise or dignified disdain. The Stoics in ancient Greece were philosophers who believed that self-control is the highest virtue, and detachment from strong emotions and passion would give them greater insight in their quest for truth. They also thought that emotional reactions to the inevitable were silly; given that We All Die Someday, what is grieving over death but a judgment that the inevitable was somehow wrong? Stoics would later be criticized for fatalism and apathy.
The Voiceless: A character isn't shown speaking, but might still be capable of speech.
Tranquil Fury: This character can range from happy or stoic, but their anger is more quiet (but still dangerous). What defines this trope is the tendency to become deadly serious when it gets deadly serious.
Examples
In the story "The Six Swans", collected by the Grimm Brothers and Hans Christian Andersen among others, a Fallen Princess must make six shirts out of nettles and can't make a sound for seven years or the spell that transformed her six brothers into swans will never be broken. She manages to keep all of these conditions and gets to break the spell. This is an example of the Elective Mute trope.
Peter in Jumanji, who talks to no one but his older sister Judy ever since their parents' death by car accident. Once Alan gets out of the game and finds his parents are also dead, Peter starts talking to him as well.
Charles Wallace was an Elective Mute trope a child in A Wrinkle in Time. By the time of the later books, he has grown out of it.
Irish Mythology: The battle trance Nuada enters before the first battle of Maige Tuired is sometimes described as a battle fury. However, unlike The Riastrad, the famous "Warp Spasm" of the hero Cu Chulainn, Nuada does not become a berserker, but instead becomes exceptionally calm. This is an example of the Tranquil Fury trope.
Older Than Steam. Shakespeare's Henry V has the eponymous character's Tranquil Fury reaction to the tennis balls.
Dead Poets Society: The shy and insecure Todd Anderson spends most of the film struggling to get out two full sentences and is overlooked by the school and his parents. After his best friend kills himself, the school tries to bully him (and the other boys) into pinning the blame on their favorite teacher — and he leads half the class in an outright rebellion against the headmaster.
Don Vito Corleone from The Godfather is famously very soft-spoken, even hoarse, but an extremely menacing screen presence.
In the original novel The Godfather, both Vito and Michael Corleone were noted as young men for being soft-spoken, understated, and reasonable, especially in contrast to many of their Sicilian immigrant and first-generation compatriots. They go on to become in turn the most feared "Family" heads of their generations, while still rarely raising their voices above a normal speaking tone.

This is already quite a specific character, and it seems you have a rough idea of your storyline. I don't want to intrude too much on your story, so I compiled tropes and examples in literature as well as other media that are somewhat related to what you described, and you could perhaps incorporate these (& edit as needed/desired) to further flesh out your specific character and plot. Consider which direction you want your story to go; what reactions you want your own character to show once they're thrust into that new environment (Will they continue to be quiet? Will they go the other end of the spectrum? Perhaps somewhere in between? Will they succeed in "adapting" in this new environment?). Do go through the sources for more information and examples. Whumpee who finally started trusting Caretaker enough to doze off in their arms. But then something happens. Maybe caretaker moves too quickly. Maybe they touch Whumpee somewhere that triggers them. Suddenly they bolt upright and quickly move away with fear in their eyes. Putting as much space between them as they can while they frantically try to reassess if they're in danger again. All of that trust now gone in the blink of an eye due to just how skittish Whumpee still is.
as someone who is into languages a little too much I have discovered that "miscommunication due to language barrier and/or illiteracy" is a delicious flavor of angst that I prefer a lot more to other types of miscommunication in fiction (ie characters just not talking to each other)
HOW TO WRITE A CHARACTER WHO IS IN PAIN
first thing you might want to consider: is the pain mental or physical?
if it’s physical, what type of pain is it causing? — sharp pain, white-hot pain, acute pain, dull ache, throbbing pain, chronic pain, neuropathic pain (typically caused by nerve damage), etc
if it’s mental, what is the reason your character is in pain? — grief, heartbreak, betrayal, anger, hopelessness, fear and anxiety, etc
because your character will react differently to different types of pain
PHYSICAL PAIN
sharp and white-hot pain may cause a character to grit their teeth, scream, moan, twist their body. their skin may appear pale, eyes red-rimmed and sunken with layers of sweat covering their forehead. they may have tears in their eyes (and the tears may feel hot), but they don’t necessarily have to always be crying.
acute pain may be similar to sharp and white-hot pain; acute pain is sudden and urgent and often comes without a warning, so your character may experience a hitched breathing where they suddenly stop what they’re doing and clench their hand at the spot where it hurts with widened eyes and open mouth (like they’re gasping for air).
dull ache and throbbing pain can result in your character wanting to lay down and close their eyes. if it’s a headache, they may ask for the lights to be turned off and they may be less responsive, in the sense that they’d rather not engage in any activity or conversation and they’d rather be left alone. they may make a soft whimper from their throat from time to time, depends on their personality (if they don’t mind others seeing their discomfort, they may whimper. but if your character doesn’t like anyone seeing them in a not-so-strong state, chances are they won’t make any sound, they might even pretend like they’re fine by continuing with their normal routine, and they may or may not end up throwing up or fainting).
if your character experience chronic pain, their pain will not go away (unlike any other illnesses or injuries where the pain stops after the person is healed) so they can feel all these types of sharp pain shooting through their body. there can also be soreness and stiffness around some specific spots, and it will affect their life. so your character will be lucky if they have caretakers in their life. but are they stubborn? do they accept help from others or do they like to pretend like they’re fine in front of everybody until their body can’t take it anymore and so they can no longer pretend?
neuropathic pain or nerve pain will have your character feeling these senses of burning, shooting and stabbing sensation, and the pain can come very suddenly and without any warning — think of it as an electric shock that causes through your character’s body all of a sudden. your character may yelp or gasp in shock, how they react may vary depends on the severity of the pain and how long it lasts.
EMOTIONAL PAIN
grief can make your character shut themself off from their friends and the world in general. or they can also lash out at anyone who tries to comfort them. (five states of grief: denial, anger, bargaining, depression and eventual acceptance.)
heartbreak — your character might want to lock themself in a room, anywhere where they are unseen. or they may want to pretend that everything’s fine, that they’re not hurt. until they break down.
betrayal can leave a character with confusion, the feelings of ‘what went wrong?’, so it’s understandable if your character blames themself at first, that maybe it’s their fault because they’ve somehow done something wrong somewhere that caused the other character to betray them. what comes after confusion may be anger. your character can be angry at the person who betrayed them and at themself, after they think they’ve done something wrong that resulted in them being betrayed, they may also be angry at themself next for ‘falling’ for the lies and for ‘being fooled’. so yes, betrayal can leave your character with the hatred that’s directed towards the character who betrayed them and themself. whether or not your character can ‘move on and forgive’ is up to you.
there are several ways a character can react to anger; they can simply lash out, break things, scream and yell, or they can also go complete silent. no shouting, no thrashing the place. they can sit alone in silence and they may cry. anger does make people cry. it mostly won’t be anything like ‘ugly sobbing’ but your character’s eyes can be bloodshot, red-rimmed and there will be tears, only that there won’t be any sobbing in most cases.
hopelessness can be a very valid reason for it, if you want your character to do something reckless or stupid. most people will do anything if they’re desperate enough. so if you want your character to run into a burning building, jump in front of a bullet, or confess their love to their archenemy in front of all their friends, hopelessness is always a valid reason. there’s no ‘out of character’ if they are hopeless and are desperate enough.
fear and anxiety. your character may be trembling, their hands may be shaky. they may lose their appetite. they may be sweaty and/or bouncing their feet. they may have a panic attack if it’s severe enough.

I love the "came back wrong" trope but from the opposite side.
Imagine you are dead. And then you are RIPPED from the embrace of decay into the world of the living again. Your memories are hazy and you don't recognize any of these people, but they act like they're close to you? Like they love you? So you try to get your memories back, to act like you belong here, but everybody tries to forget you died. And you can't. It is omnipresent. And just trying to grapple with that fact pushes the people who "love" you away, and they're incapable of understanding, and they're so confused, what's wrong N̶̄̀O̶͛͗T̷̉́ ̷͋͝Y̴̎̌Ȍ̴̈U̸̓R NÄM̴̃͑E̵̾̇? And you just need them to understand, you aren't that person! You aren't! You don't know who that person is! You don't know why any of this is happening, but they're unwilling to bend, they keep insisting you are that person, your memories will come back, everything will be normal again, and you want to scream and cry and claw yourself open to show them you're different. Your existence as a being wholly separate from whoever you "used to be" is a sin unto itself. All you can do is scrabble for life and to them, you're killing whoever they loved to do it.
