What if there's an Old wives tale in Fleabottom about how Omegas that have complementary scents can stabilise a pregnancy. Like having an omega companion when you have high risk of still borns and miscarriages have a higher chance of their children growing to term and surviving.
Now imagine Kiera and Duncan become friends and Kiera explains her pregnancy fears. So Duncan ask if she wants him to stay and have scent sessions to help with her pregnancy as he heard it can help.
When she doesn't answer immediately Duncan starts to apologise profusely but Kiera has tried nearly ever highborn 'cure' to help her birth a healthy babe and it hasn't worked; so out of desperation she agrees.
They and Valarr (+Egg) go to Dragonstone for the rest of the pregnancy. This time Kiera does give birth to a healthy boy.
She and Valaar are besides themselves with joy and start profusely thanking Duncan even though he says its no problem.
Cue whenever Kiera gets pregnant, usually after the first 2 moons, Duncan will get a raven asking him to come to Dragonstone and help support her.
Maybe it even gets to the point where on an adventure Duncan just stops, smells the air and squints, before telling Egg they need to go to kings landing so he can help Kiera with her latest pregnancy.
When at the Red Keep Kiera is shocked to see them but even more baffled when the Maester confirms she is pregnant.
This of course doesn't stop her from having miscarriages but her and Valarr are able to have 3 sons and a daughter and its more than Kiera could ever ask for.
