Here is a Chihuahua patient with pharmacologically confirmed partial Horner syndrome of the right eye (ie miosis mild ptosis). This patient presented with a history of having a small than normal right eye. This was an illusion given the ptosis. Horner syndrome results from anywhere along the sympathetic nervous system pathway to the eye. Complete Horner syndrome, in the dog, is characterized by ptosis, enophthalmos, miosis, and protrusion of the third eyelid. Pharmacological testing confirms the diagnosis. CullenWebb recommends that all patients with unexplained changes in pupil size or changes in the appearance to the eye be referred to a board-certified veterinary ophthalmologist. Here is the left eye of a young adult cat that was referred with concern about a black mass that was noticed in the eye during a dental cleaning. This patient has pigment deposition on the anterior lens capsule (from the posterior iris epithelium) associated with a pinpoint scar within the cornea and inline with an area of focal fibrosis and pigment on the lens capsule and pinpoint anterior cortical and nuclear cataract. These findings point toward a previous penetrating eye injury. The owners were unaware of an injury ever occurring. Nonetheless, the eye appeared comfortable and had no evidence of uveitis. This is an example of the importance of referral of patients with ocular abnormalities that cannot be explained. Here are photographs of a mature, female feline patient seen at CullenWebb with feline aqueous humor misdirection syndrome (FAHMS). FAHMS is a rare and insidious form of glaucoma in the cat. The condition presents asymmetrically, with anisocoria characterized by dilation of the glaucomatous eye and with a uniformly narrowed anterior chamber, as was the case in this patient. The left eye of this patient is now becoming affected too. Intraocular pressures are slowly becoming elevated despite appropriate medical management. In the photos of the affected patient you can appreciate a narrowed anterior chamber (right eye more narrowed than left eye), compared to the normal anterior chamber of an unaffected cat (lower two images). Surgical therapy may be beneficial if performed soon in the course of the disease. Here is the eye of a 6 month old Peterbald cat 🐱. In the upper photo you can appreciate the entropion (rolling in of the eyelid margin) of the lower eyelid. In the lower photo you can appreciate the extent to which the eyelid needed to be rolled out to see the hairless margin. Both eyes were affected similarly and this patient. This patient was reluctant to hold his eyes open because of the pain associated with the entropion. Surgery was performed and it is anticipated that this patient will be happy and pain-free going forward. Entropion is uncommon in cats compared to dogs, except in brachycephalics that not uncommonly have medial entropion. Nonetheless, entropion can be breed related, secondary to eyelid malformations or infectious diseases. It is important to recognize that hairless breeds such as Sphynx and Peterbalds are not immune to the deleterious effects of entropion. These breeds too have haired (albeit small hairs) along the skin bordering the margin of the eyelids. As such, cats with entropion can suffer from pain during simple blinking because it is analogous to sandpaper rubbing on the eye (or third eyelid) every time the patient blinks. As such, the underlying cause, if any, of the entropion must be identified and treated appropriately. In most instances surgical correction is required. In instances where entropion is breed-related, the breeder should be made aware so as to potentially adjust breeding practices. CullenWebb recommends that feline patients with entropion be examined by a board-certified veterinary ophthalmologist so as to identify any concurrent underlying cause (eg infectious causes) and to provide expert, tailor-made diagnostic and treatment options. This is the eye of an older Shih Tzu patient seen at CullenWebb. This patient was referred for cataracts. You can appreciate the white speckled appearance within the pupil. These represent a condition known as asteroid hyalosis. Asteroid hyalosis represents a degenerative condition involving the gel in the back of the eye (ie vitreous). The white spherical bodies are known as asteroid bodies and are calcium-lipid complexes. Such a finding on distant examination should not be confused with cataracts.
