Honestly the thing that helps me (personally, this isn't professional) the most with big overwhelming care tasks that seem impossible is honestly setting the bar as low as possible. Dishes piled up? I'll take literally only one thing when I go to the kitchen for something. Can't bathe and feeling gross? Use bath wipes for some immediate improvement. Too exhausted to work out? Do one single lie-down or seated stretch. Can't clean everything? Focus one tiny area only.
Be reasonable with yourself about your expectations. No "I should be able to do this". One thing is better than no things. It counts. Plus even if it's small it can prove to your brain that you can in fact accomplish things and a quick win rules when you're down.
