Worst part is that if you asked Doc Garaki of all they've done to Tomura, he wouldn't remember the details because he has committed so many crimes against humanity that Tomura's one are just part of the pile.
You can't ask Tomura because he still thinks it was all his fault, at least partially.

Idk, something about the horror of knowing that Tomura was with AFO and Doc Garaki for 15 years and there's no way to know what they did to him. Just to put it in perspective, the pro-heroes don't even guess about the Shimuras tragedy and that was the main event of Tomura's trauma. Do you think they can even imagine what other horrors Tomura faced in those 15 years?
Even if AFO did nothing. Even then, you can be certain that he tortured and killed a child a month and went "home" thinking he couldn't wait for the time it was Tomura's turn. He filled those children with drugs and experimented on them and broke them like dolls and forced their minds and bodies to stay alive through his command, AFO did that constantly— and then he bought Tomura a new toy and told him he was special.
