My first Odontectomy as Dentistry ClerkShip at Department of Oral Surgery Gadjah Mada University, Id. With diagnosis of Horizontally Inclined , Class I A element #48 The treatment planning were : 1. Odontectomy 2. Medication 3. Re-Evaluation Before the odontectomy procedure the patient was anesthesized by mandibular nerve block and with the addition of n.buccalis longus anesthesia infiltration. The odontectomy was started by incision from distal #48 up to 1/3 distal #46 , creating a triangular flap, and make sure that the incision also cut the periosteum before dissecting the flap with raspatorium. When already have the clear vision to the bone and the #48, reduced the buccal bone with low speed handpiece with round bur for bone until the bifurcatio of the teeth was exposed. Continue with separating the teeth with low speed handpiece and fissure bur, with the direction from bifurcatio to the crown, stop the burs when it's possible to elevate the mesial section of the teeth. When the mesial section is already extracted, continue by elevating the distal section from under the distal crown using distobuccal motion of the elevator, levering the distal section. After the distal section was extracted, make sure that there is no root parts left behind. The next step is curretage, eliminate the sharp margin of the bone, and clear the socket with sterile saline, put 2 Spongostan into the socket, And then the extraction was finished with 4 Simple interrupted suture.
