Susan McGowan from North Lanarkshire died two weeks after taking the drug tirzepatide, brand name Mounjaro.
The death of a nurse from North Lanarkshire has been linked to the use of a weight-loss drug recently approved for use on the NHS.
Susan McGowan, 58, took two low-dose injections of tirzepatide, known under the brand name Mounjaro, over the course of about two weeks before her death on 4 September.
Her death certificate, seen by the BBC, lists multiple organ failure, septic shock and pancreatitis as the immediate cause of death – but "the use of prescribed tirzepatide" is also recorded as a contributing factor.
There have been 23 suspected deaths linked to semaglutide in the UK via the yellow card scheme since 2019.
BBC News asked the department of health and social care if it would review its decision given Ms McGowan’s death.
The UK government declined to comment.

I keep thinking about how easy it was to get a prescription for a glp1. Like maybe too easy like I feel like they should have screened me more. Obviously I’m glad they didn’t because I wanted the prescription, but it felt a little bit irresponsibly easy. This is the smoothest an interaction with the medical system has ever gone for me, and big surprise, it’s on the one that makes me less fat. Maybe my real feeling is just that this is maybe my first brush with the medical system working properly and quickly, and I’m just cynical about the fact that its on the most inconsequential thing, and on the thing that upholds societal fatphobia. I wish every medication was as easy to get as ozempic forever
