Xanxus realized young and Timoteo supported him wholeheartedly and got him on hormone blockers when he could and still personally pays for his testosterone. Xanxus paid for his own surgeries though.
Squalo knew how he felt young, but didn’t care that much. It wasn’t until he joined the Varia that he started testosterone and he got himself top surgery a few years after Xanxus got frozen.
Bel is honestly probably closer to transmasc nonbinary, but he doesn’t care enough to dwell on that. He just knows he didn’t like presenting femininely or going by she/her and those are his only strong feelings on the matter.
Levi didn’t fully understand how he felt when he was young and was nervous to express it. It wasn’t until he realized he was way more comfortable with having hirsutism than other women in his family that he fully came to terms with his feelings. From there, he made sure there was no other underlying health concerns before deciding to fully transition, starting socially first.
Lussuria! Firstly I wanna say I adore and see the trans femme headcanons for Lussuria, but I also love playing with this thought experiment.
Lussuria probably never dwelled on his feelings that hard when he was young, just assuming that it’s how everyone feels. Because to him, his want for a more built and muscular body type was just his dream body he wanted to achieve through working out. He also was never really distressed about his body, just felt like he needed to work harder to eventually achieve it. It wasn’t until he was older and interacting with more queer people he realized what he was actually feeling. Because he was never inherently dysphoric, he’s still perfectly comfortable with feminine terms and doesn’t mind being called things like “Big Sister” by his subordinates or jokingly referring to himself as “the mom” of the Varia executives.
