Ablepharon Macrostomia Syndrome (AMS for short) is an extremely rare genetic disorder that severely impairs quality of life.
Unilateral Macrostomia Correction through Vermillion Return Flap and skin tag removal This little girl was born with hemifacial microsomia with unilateral macrostomia. There were also associated preauricular and macrostomia skin tags. Her parents presented to our hospital for corrective surgery. Treatment planning was explained to them and they consented to surgery. The new commissure was marked to ensure symmetry with the normal side. Orbicularis oris was reconstructed using the vermilion return flap. Repair was performed in three layers using the muscles, subcutaneous tissue and skin. This would result in negligible scar formation. Dissection and excision of the skin tags was also performed to ensure that there would be optimal esthetic results. She will now be able to speak clearly and eat without dribbling. Her appearance was also completely normalized with establishment of symmetry. Condylar Grafting with Rib Perichondrium Growth Center This preteen was born with hemifacial microstomia and has already undergone surgery for correction of macrostomia and removal of ear tags at our hospital when she was 10 months old. She now presents with a facial asymmetry due to a hypoplastic right condyle. Treatment planning was explained to her parents and they consented to surgery. She underwent augmentation of her hypoplastic condyle with a grafting of rib costochondral junction with perichondrium growth center. This resulted in correction of her facial asymmetry. The growth center will ensure that there is normal growth of the condyle henceforth. Macrostomia – Bilateral Reconstruction of Angle of Mouth This little girl was born with an abnormally wide mouth, which was diagnosed as macrostomia. Her parents were advised to get this addressed at a specialty facial deformity correction hospital. Their extensive inquiries led them to our hospital for corrective surgery. This deformity arises from facial clefts that can form on either side of the face. It very rarely occurs as a part of hemifacial microsomia. Surgical correction of this deformity not only normalizes facial esthetics, but also helps in development of normal speech. Dr SM Balaji, facial reconstruction surgeon, examined the patient. She had bilateral macrostomia with involvement of bilateral modiolus. He explained the treatment plan to the parents in detail and they consented to surgery. Markings were first made to ensure that surgery would establish symmetry of both sides of the mouth. Orbicularis oris muscle was identified and reconstructed. This was followed by identification and reconstruction of the buccinator. A vermillion return flap was then raised to reconstruct the vermillion and ensure mucosal blending with the skin at the commissures of the mouth. Three layer closure was finally performed to ensure that there was good balance between the contour of the mouth and cheek skin. Her parents were extremely happy with the results of the surgery. This recreated perfect lip morphology with good functional results.
